Step 6 - Application Summary and Application Submission ("Declaration Form" signed on paper)

Review the application summary carefully. Select "Edit" to go back to the respective step to amend the relevant information if necessary.

Click the radio button to declare whether you are bankrupt and/or applied for Individual Voluntary Arrangement.

Select the method of signing the "Declaration Form". You should select "Sign on Paper" if you do not have an "iAM Smart+" account. You have to download and print the "Declaration Form" in Step 7 and sign the "Declaration Form". Please submit the duly signed "Declaration Form" together with the required supporting documents to the SFO.

Select the method of submitting the "Declaration Form" and supporting documents (through online upload, by mail or through the SFO drop-in boxes) and select "Submit".

Once the online application is submitted, no online amendments will be allowed. All subsequent amendments should be made in writing to the SFO if necessary.
