Complete the mandatory fields marked with “*” under each step. You cannot continue to apply if you fail to provide the mandatory information.

Input the names shown on your HKID Card. Please leave a space between each word for the English name. If a Chinese name is recorded in your HKID Card, the field “Name in Chinese” must be input.

If you have the right of abode in Hong Kong Special Administrative Region of the People’s Republic of China (“HKSAR”), an alphabet “A” should be shown below the Date of Birth on the front side of your HKID Card. If you do not have the right of abode in the HKSAR, you should also provide a copy of your entry permit to the HKSAR, e.g. One-way Permit / dependant visa, etc., with your application.

Fill in residential address.

Also fill in correspondence address if it is different from residential address.
