Step 9 - Download the Application Summary with Declaration (for Declaration Sign on Paper), Undertaking, Deed of indemnity and Checklist for Submission

Select "Download" to download and print the cover letter, Application Summary with Declaration (Except applicants who have already signed the Declaration digitally via “iAM Smart+”), Undertaking, Deed of Indemnity and Checklist for Submission.

Use unzip software and input password to extract the download file. The password is the first 6 characters of the applicant’s HKID card number.
